Checklist for Your First Visit
Please arrive 15 minutes early to complete any remaining paperwork. Remember to bring:
[✓] Your current Health Insurance Card.
[✓] A valid Photo ID of the parent/guardian.
[✓] Your child’s Immunization Record (Shot records).
[✓] Previous medical records (if transferring from another doctor).
[✓] List of current medications (if any).
FAQs
Do you offer same-day sick visits?
Yes, we reserve time daily for sick visits. Please call us as early as possible in the morning to schedule.
Can I meet the doctor before my baby is born?
Absolutely! We encourage expectant parents to schedule a complimentary “Meet and Greet” visit to tour our office and meet our providers.
What is your cancellation policy?
If you are unable to keep an appointment, we ask that you kindly provide 24 hours notice.
