Sagebrook Pediatrics is a fictional practice, and this page is part of a design portfolio. It follows the structure a real practice needs, but it is not legal advice and covers no real patient. About this site.

Who this policy covers

This policy covers this website. It explains what we learn about you when you visit it or send us a request through it.

Your child’s health information, the records we keep as your child’s doctors, is protected by federal law and covered by our Notice of Privacy Practices.

What we collect

When you send a visit request

The form on our Contact page asks for:

  • Your name and phone number
  • Your email address, if you choose to give it
  • What the request is about, your child’s age range and the best time to call, if you choose to give them
  • Anything else you type in the message box

When you visit any page

Like every website, ours is served from a web host. The host’s servers record basic technical details for each request: your IP address, your browser type, the page asked for and the time. These logs are used to keep the site secure and working, and the host keeps them only for a short period.

What we do not collect

  • No analytics or visitor tracking
  • No advertising or social media pixels
  • No accounts, logins or patient portal

Please keep health details off the form

The visit request form is for booking and general questions. It is not a private channel for health information. Please do not use it for symptoms, test results, medication questions or anything urgent. Call (541) 555-0148 instead. After hours, the same number reaches one of our pediatric providers. In an emergency, call 911.

How we use what you send

We use your request to call you back about it, and for nothing else. We do not sell it, rent it, share it for advertising or add you to a mailing list.

Two kinds of service providers handle it for us: our web host, which runs the site and keeps a short log of the emails it sends, and our email provider, which delivers a copy of each request to the front desk. They process it only to provide those services.

We may disclose information if the law requires it, or to protect someone’s safety.

Cookies and embedded content

This site sets no tracking or advertising cookies. The typefaces are served from our own site, not from a third party.

Our Contact page can show a map provided by Google. It does not load on its own: the page shows a picture of the map, and Google’s map loads only if you choose Load map. When it loads, Google receives your IP address and may set its own cookies, under Google’s privacy policy. The address and a directions link are on the same page as plain text if you prefer not to load the map.

Members of our staff who sign in to manage the site receive the login cookies WordPress needs. Visitors never do.

How long we keep it

  • Form requests are not saved by the website. The website’s email log keeps a copy of each request for 30 days, then deletes it automatically.
  • Email copies of a request are deleted once it has been handled, and within 90 days.
  • Anything that becomes part of your child’s medical record, such as an appointment, is kept as medical records law requires, and is covered by our Notice of Privacy Practices.

How we protect it

Every page is served over an encrypted connection (HTTPS). Only front desk staff can read form requests. No website can promise perfect security, which is one more reason to keep health details off the form.

Children

This site is written for parents and guardians. We do not knowingly collect information from children under 13 through it. If we learn that a child has sent us a request, we delete it.

Your choices

You can ask us what we hold from a request you sent, and ask us to correct or delete it. Call or email us and we will answer within 30 days. Where a state privacy law, such as the Oregon Consumer Privacy Act, gives you further rights over your personal data, we honor them. We do not sell personal data or use it for targeted advertising.

Changes to this policy

If we change this policy, we will post the new version here and update the date at the top of the page.
